Coaching Agreement between CLIENT and Dr. Kustec Coaching (Vanessa Kustec M.P.C.) 

Coaching agreement between Vanessa Kustec M.P.C. (“Coach”) and CLIENT for personal coaching to be provided to CLIENT.

TERM

This Agreement shall begin on the CLIENT's start date and shall continue for a period of 6 weeks (for 1:1 Coaching - Mini) or 12 weeks (for 1:1 Coaching - Full and Group Coaching).  

SCOPE OF SERVICES

The services provided will include but are not limited to: 

  • Coaching on key topics to help achieve personal +/- professional goals and develop optimal work-life balance.
  • The Coach will provide hands-on help and support for questions and challenges. The Coach will supply reading materials, high-quality strategies and tools for immediate implementation, and action steps to practice weekly for quick results. 
  • The Coach also provides continual email support/coaching to create a cushion of support. 

MEETINGS

The Scope of Services shall be achieved through: 

1:1 Coaching - Mini

  • 6 scheduled calls over 6 weeks of 60 minutes long (or longer if needed). Any team member may be on a call at any time and the calls may be recorded. 

1:1 Coaching - Full and Group Coaching

  • 12 scheduled calls over 12 weeks of 60 minutes (or longer if needed). Short “booster” calls in between sessions if needed. 
  • For group coaching only: one initial 1:1 call at the start of the course.
  • The coach will get back to the client as soon as possible at all times.

RIGHTS AND RESPONSIBILITIES OF COMPANY

CLIENT agrees to: 

  • Be available for scheduled calls 
  • Give Coach at least 24 hours-notice to reschedule our coaching session if needed.  Missed appointments will not be made up absent extenuating circumstances
  • Come to coaching sessions prepared for a great session and do their best to be focused
  • Complete all assigned tasks whenever possible 
  • Increase willingness to try new things suggested by Coach

PAYMENT

CLIENT agrees to have credit card charged for payment to Vanessa Kustec as follows: 

For group coaching:
- Single payment of $2400 CAD + HST or
- 3 monthly payments of $880 CAD + HST

For 1:1 coaching - full:
- Single payment of $3000 CAD + HST or
- 3 monthly payments of $1100 CAD + HST

For 1:1 coaching - mini:
- Single payment of $1800 CAD + HST or
- 3 monthly payments of $660 CAD + HST

INDEPENDENT CONTRACTOR 

Coach is an Independent Contractor and nothing herein shall be construed to create an employment agreement, or any other agency relationship between Coach and the CLIENT.  All work created under this Agreement shall be property of CLIENT. 

  ASSIGNMENT 

Coach agrees that Vanessa Kustec M.P.C. will provide the agreed-upon services above and that such Services may not be assigned to any third parties. 

NOTICE:

I CONFIRM THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT PRIOR TO ACCEPTING IT. I AM AWARE THAT BY CHECKING "I HAVE READ AND AGREE TO" THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, ASSIGNS OR REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES, INCLUDING THE RIGHT TO SUE OR TO RECEIVE ANY COMPENSATION FROM ANY OF THE RELEASEES.
